Mosaic Family Health is co-sponsored by Ascension Wisconsin and ThedaCare, two local health systems that provide resources like physician mentors and teaching hospitals. Our partnership with Ascension Wisconsin and ThedaCare allows our doctors to care for you at ThedaCare Regional Medical Center – Appleton and Ascension St. Elizabeth Hospital, as well as collaborate with other community physicians to deliver exceptional, holistic care. - Medical College of Wisconsin

As part of our commitment to provide quality care for our patients at every stage of life, we partner with local nursing home Brewster Village. Our physicians visit and monitor care for our patients who require short-term rehabilitation or long-term skilled nursing services at Brewster Village.
