Kimberly Goffard, MBA/HCM, RN, Education Coordinator, joined the Fox Valley Program in 2007. Kimberly earned her BS in Nursing at the University of Wisconsin-Oshkosh and her MBA with Health Care Management emphasis through the University of Phoenix-Fox Valley campus. Kimberly brings 20 years of healthcare experiences to the Fox Valley Program. Kimberly and her spouse Dan, have three boys and she enjoys dancing, traveling, and cheering for her sons at their sporting events.
Beth Belmore, MSE
Job Title: Clinic/Community Care Coordinator
Beth has a Bachelors in Education, graduate level training in Marriage and Family Counseling from Northern Illinois University and a Masters in Counseling with a community focus from UW-Oshkosh. Starting at the program in the fall of 2013, she brings over 25 years of leadership of community based and residential experience in social services. She is a resource for accessing needed community resources and supports the provision of collaborative physical and mental health care with social services. Her community health and teaching interests include advanced care planning and memory loss. She represents Health Fox Valley on the Fox Valley Memory Project and the Fox Valley Coalition for End of Life Care as well as other community boards and projects. Beth is married and has one adult daughter. She enjoys journaling, travel, and outdoor adventure.
Suzanne Blazek, MSOLQ
Job title: Assistant Education Coordinator/Recruitment Coordinator
Suzanne joined the Fox Valley Program in March 2015. She earned her BS in Human Resource Management and Master's of Science in Organizational Leadership and Quality from Marian University. Her past career included Physician Recruitment and Human Resources. Suzanne is repsonsible for coordinating orientation, resident rotations, curriculum development and the recruitment efforts for the program. Suzanne enjoys spending time with family, exploring Wisconsin, boating and fishing.
Teri Metropulos, MA, LPC-IT
Job title: Behavioral Health Counselor
Teri is a State Certified Social Worker. She received her Bachelor of Science degree in Psychology from the University of Wisconsin La Crosse and her Master’s Degree in Counseling from Lakeland University.
Teri has worked as a State Certified Social Worker in the Fox Valley since 1990. Her work has spanned all generations, from birth to end of life. Her career has included work as a child welfare social worker, a State of Wisconsin treatment foster care administrator, a juvenile justice mentoring coordinator, manager of a non-profit organization for the Criminal Justice System, called Clean Break and spent 12 years working in End of Life Care as both a clinician and a Manager. Through her work experiences, Teri has found that her passion lies with supporting and empowering individuals to achieve their optimal level of mental health.
She and her family are avid Badger fans and spend time volunteering for their church and the community. Teri enjoys running, cooking, and knitting as well as finding any great venue to listen to music of all genres.
