Planned leadership transition positions Mosaic to build on its mission of community centered care.
APPLETON, Wis., Jan. 22, 2026 – Mosaic Family Health has named Leif Elsmo as its new president following a planned leadership transition designed to ensure a continuity of care for patients across the Fox Valley. Elsmo will step into the role on Jan. 26, 2026. Mosaic Family Health is committed to providing comprehensive, mission-driven care to more than 6,000 patients in the Fox Valley.
“We are incredibly fortunate to welcome Leif Elsmo as Mosaic’s next president,” said Dr. Lee Vogel, president of Mosaic Family Health. “His talent and deep passion for community health will be tremendous assets to the Fox Valley. I look forward to working closely with Leif during this transition.”
Elsmo brings more than two decades of executive leadership experience in healthcare, academic medicine and nonprofit sectors. His background includes developing nationally recognized community health and health equity initiatives that integrate clinical care, public health and social services. His leadership style emphasizes collaboration and partnerships, aligning with Mosaic’s mission to deliver holistic, patient-centered care and its role as the clinical home of the Fox Valley Family Medicine Residency Program.

Vogel will support the transition through Q1 2026, ensuring Mosaic’s tradition of quality care and strong community engagement continues uninterrupted. This leadership change reflects the organization’s long-term commitment to the Fox Valley, providing family-like, patient-centered care while building on its momentum and confidently preparing for the next chapter. Following the transition, Dr. Vogel will continue in a part-time role with Mosaic, supporting resident education in the Fox Valley Family Medicine Residency Program and caring for her personal panel of patients at Mosaic.

Incoming President, Mosaic Family Health
Leif Elsmo brings more than 20 years of executive leadership experience across healthcare, academic medicine, and nonprofit organizations. Leif’s career has been defined by leadership in healthcare delivery and strategic planning, with an emphasis on health equity.
Prior to joining Mosaic, Leif served as President & CEO of Centers for Independence (CFI) and the MCFI Foundation in Milwaukee. During his tenure, CFI grew into a comprehensive health and human services organization with more than 800 employees serving over 25,000 children and adults annually through 29 programs. He also led the MCFI Foundation through significant growth, expanding its capacity to support inclusive and healthy communities for people with disabilities across Wisconsin. Central to this work was CFI’s commitment to total health, integrating technology and innovation to improve outcomes for individuals and families.
Earlier in his career, Leif held senior leadership roles at UChicago Medicine, where he assisted development of the Urban Health Initiative into a nationally recognized model for advancing health equity. His work supported the creation of comprehensive trauma and violence recovery programs in underserved communities.
Leif holds an MBA from the University of Chicago Booth School of Business and bachelor’s degrees in political science and philosophy from the University of Wisconsin–Milwaukee. Outside of work, he enjoys spending time outdoors and attending live music events with his wife, Mel.
