Mosaic Family Health provides access to high-quality clinical care for more than 6,000 patients at its convenient location in downtown Appleton. For the last 30 years, the clinic has provided holistic care and support for people of all ages. We offer a wide range of services, including maternity care, pediatrics, acute and chronic illness care, geriatrics, mental health care and hospital care.

Teaching the art & heart of care
Mosaic Family Health is home to the Fox Valley Family Medicine Residency Program, an affiliate of the Medical College of Wisconsin. We work together to fulfill our mission: to provide full-scope family medicine services while training future physicians and improving the overall health of the Fox Cities.
The longstanding clinic is co-sponsored by two local health systems, Ascension Health Care and ThedaCare. Our sponsors provide community physicians and hospital training sites to support our freshly schooled doctors. Mosaic Family Health also partners with other community health organizations to help patients and their families through every aspect of care. We provide services at:
