Faiza Aziz, DO

Faiza grew up in Twin Cities, Minnesota, where she received her undergraduate and master degrees in Biology from University of Minnesota-Twin Cities. She then pursued medical training at Ohio University-Heritage College of Osteopathic Medicine in Athens, Ohio.
Faiza was introduced early on to community family medicine, while growing up in rural Kenya. Then again, while at Hennepin Healthcare in the Twin Cities in various capacities as an infectious disease social worker, as well as a multilingual medical interpreter.
She learned the connections between social determinants of health, underrepresented communities (rural and urban), and health disparities. These experiences led her to pursue clinical medicine, with the intention of being a bridge to lessen disease burden while serving locally, but still keeping her global perspective. Having a holistic view of patient health is what has led her to family medicine, as well as it’s potential to establish lasting rapports with patients, policy collaborations, and an abundance of community interface.
When she isn’t working, she enjoys being with her family, traveling to bucolic unconventional or culturally diverse places, listening to world music, practicing yoga, and watching track and field games.
Faiza was drawn to the Fox Valley Family Medicine Residency because of the instantaneous welcoming interaction from the interviewing faculty team, as well as other residents and staff. It aligned perfectly with her vision of serving the community broadly, as well as the strong educational support she felt as a resident.

Domose Enodunmwenben, MD
Domose Enodunmwenben, MD

MD: All Saints University School of Medicine, Dominica
Pre-Med: Avalon University School of Medicine, Curacao
Domose was raised in Southern Nigeria. Inspired by her mother’s career as a physician and her father’s leadership of a nonprofit organization, she developed a deep commitment to serving others through medicine. Completing her Premedical education at Avalon University School of Medicine, Curacao, this served as the foundation for her medical training at All Saints University School of Medicine, Dominica. Her interest in family medicine was developed early on through participation in health fairs and outreach events. By experiencing the tangible impact that healthcare can have on a community, it deepened her interest and reinforced her belief that by caring for individual families, we can effect meaningful change within the broader community. Some of her passions include global health, improving health access for the underserved, reproductive, and mental health.
hen she is not working, she enjoys spending time with family, playing ping pong, taking walks by the lake, and going to the gym. She also enjoys binge watching tv shows and cooking traditional Nigerian foods.
She was drawn to the Fox Valley Family Medicine Residency because of its full-scope training and extensive opportunities for community engagement, recognizing it as the ideal environment to grow as a well-rounded family physician and to serve communities through family-centered care.

Joseph Harter, MD
Joseph Harter, MD

MD: Medical College of Wisconsin
BS: University of Wisconsin-Madison
Joseph grew up in Hartland, Wisconsin, and earned his undergraduate degree in Materials Science & Engineering at University of Wisconsin-Madison, then pursued his medical training at Medical College of Wisconsin, as well as a preliminary year in general surgery at the University of Vermont Medical Center. His path to medicine has been shaped by the dedication of physicians who treated his family, a passion for mentorship, and a fascination for the intricacies of the human body.
Outside of medicine, Joseph enjoys a variety of outdoor activities which include sailing, hiking, skiing, kayaking, rock climbing, and lap swimming. Whenever possible, he enjoys traveling abroad with family, where they get immersed in the local cultures and natural wonders of the world. Creative endeavors include aquascaping, woodworking, and embracing his role as a goofy uncle.
He was drawn to the Fox Valley Family Medicine Residency for its exceptional training, which will prepare him to effectively serve rural Wisconsin communities. The versatility of family medicine is exciting to Joseph, offering the flexibility to tailor his career interests and adapt to his patient’s ever evolving needs. He’s looking forward to growing within a program that embodies leadership, humility, and patient-centered care.

Madeline Lenz, DO
Madeline Lenz, DO

Madeline grew up in Freedom, Wisconsin and then went on to pursue her undergraduate degree in biomedical sciences at Marquette University in Milwaukee, Wisconsin. She was inspired to pursue medicine by her dad, who worked as a primary care physician in internal medicine. Madeline chose to pursue medical training at A.T. Still University, Kirksville College of Osteopathic Medicine. Early in her medical training, she worked with multiple family physicians who inspired her to pursue this specialty. She loves the variety of patient care she can provide, as well as holistic and personal care, while creating lasting relationships with patients.
When she is not working, she enjoys spending time with her fiancé and family, especially her nieces and nephews. She loves exploring new food options in the Fox Valley, getting outside to walk, bike, or ride horses, and she loves to read.
Madeline was drawn to the Fox Valley Family Medicine Residency because of the holistic care that the residents and faculty can provide to entire community. She loves that residents are surrounded by well-trained, full scope family physicians while being able to make a real difference in her own hometown.

Matthew Reader, MD
Matthew Reader, MD

Matthew grew up in Appleton, Wisconsin. He received his undergraduate degree in Human Biology at Chadron State College in Nebraska, where he also played football and basketball for the Eagles. Matthew has always had a strong interest in health and wellness, and a strong desire to help those in the community around him, is ultimately what led him to a career in medicine. He then pursued his medical training at Trinity School of Medicine. Early in medical training, he had the opportunity to work as a medical scribe with a primary care physician in the Milwaukee area, and this drew him to the broad scope of family medicine and the ability to build strong relationships with patients.
When he is not working, Matthew enjoys spending time with his wife and son, and their yellow lab. He likes to stay active by playing golf, basketball, and home projects. Being an avid sports fan, Matthew follows the Bucks, Brewers, Packers, and the PGA Tour.
Matthew was drawn to the Fox Valley Family Medicine Residency because of the great opportunity to come home, to continue to learn, and give back to the community he was raised in.

Adrian Theriot, MD
Adrian Theriot, MD

MD: All Saints University, School of Medicine, Dominica
BS: University of Nevada-Las Vegas, College of Southern Nevada
Adrian grew up in Leesburg, Florida until her senior year of high school, when her family moved to Las Vegas. It was there that she completed her pre-requisites for medical school at University of Nevada-Las Vegas and College of Southern Nevada. She then pursued her medical training at All Saints University, School of Medicine, Dominica.
Outside of practicing medicine, Adrian enjoys reading novels, cooking, and playing tabletop games. She and her husband also enjoy ballroom dancing and spending time with their twelve-year-old dog, Beauty.
Adrian was drawn to the Fox Valley Family Medicine Residency because of the overwhelming enthusiasm she witnessed during interview day, and this is what set Fox Valley Family Medicine Residency apart from all other programs. The opportunity to learn full-scope family medicine care in preparation to one day open her own private practice is what brought her to choose Mosaic as her final landing place for residency.

Anne Wallace, DO
Anne Wallace, DO

DO: Kansas City University College of Osteopathic Medicine
BS: Iowa State University College of Engineering
Anne grew up in Johnston, Iowa. She earned her undergraduate degree in Chemical Engineering with a minor in Biology. Engineering provided her with a valuable STEM knowledge base and strong problem-solving skills, but she found herself more fulfilled by her interactions and relationships with people than what the engineering world provided. While traveling to Pavia, Italy on a medical fellowship program a friend had recommended, she found her passion for medicine. Anne eventually decided to pursue medical training at Kansas City University College of Osteopathic Medicine in Joplin, Missouri. Throughout her clinical training she was continually drawn back to family medicine because of its ability to establish impactful relationships with patients, and provide the perfect blend of patient education, continuity of care, and the opportunity to foster a strong connection.
When she is not working, Anne likes to spend time outdoors whether that be downhill/cross-country skiing, walking, hiking, or biking. She also loves to travel, try various restaurants, and spend quality time with friends and family.
Anne was drawn to the Fox Valley Family Medicine Residency because of the broad scope approach of the program. In addition, after interviewing and meeting with faculty and residents she believed this was a community she would excel in.
