Mosaic Family Health is the clinical home of the Fox Valley Family Medicine Residency Program.

Its patients represent a well-balanced mix of medical concerns, economic and cultural groups, and ages. The clinic’s 75 staff members include family medicine faculty, 21 residents, geriatric nurse practitioner and a care manager.

In December 2024, our newly designed, state of the art clinic opened.  It is located in downtown Appleton, approximately halfway between ThedaCare Regional Medical Center-Appleton and Ascension NE Wisconsin St. Elizabeth Hospital. The clinic now features:

• Over 31,000 square feet
• 28 exam rooms including:
  • Procedure rooms
  • Osteopathic treatment rooms
• On-site laboratory
• On-site x-ray
• On-site obstetric ultrasound
• EPIC electronic health record
• Design that optimizes team-based care and space for group visits
• Expanded mental health suite
• Community partner areas
• Community kitchen
• Quiet work area and social area specially designated for resident use
• Multiple conference rooms for didactic learning
